Photography Consent Policy

What this consent covers

Most of the work we do at Skin Essentials is visual: 

  • we take photos in clinic for record keeping and note taking purposes 

  • progress photos in clinic also allow for record keeping of any changes, complications and the like.


As a visual industry, we also rely on the generosity of our patients to allow us to share in clinic, your results with other prospective patients. 

  • The aim is to show potential outcomes they may be seeking. 

  • Lastly, for the purposes of results at conferences and on social media, we will always check with you if we can share your results and show you the final result for your approval before posting anywhere. You need not agree to this, but it would help us if you did.

I have been asked by Dr Imaan Joshi, for Skin Essentials, to consent to my image being used as part of clinic materials about the effects of my procedure. 


These images are additional to those that are stored in my patient record to document the outcomes of my procedure


Instead, the images may be made available to current and prospective patients. My image will be used without any filter or modification; there will be no identifying information on these, and will be done respecting my privacy eg if I wear hijab, these won’t be shown to men. 


I acknowledge and agree that:

 


Upon receiving this withdrawal, we will:

  • acknowledge your wishes; and

  • promptly stop using your image (by deleting them from advertising).


I have read the above and consent to use of my image for these purposes as discussed.
